  • ORGANIZING

    ENTITY

    The Vermont Department of Health serves as HMG VT's organizing entity and provides a structural home that is essential for long-term system sustainability. VDH provides managerial and fiscal oversight and helps identify and coordinate partners into a leadership team that guides the HMG system as it evolves.

  • STATEWIDE

    EXPANSION

    Initiating HMG VT at the state systems level and having a statewide vision from the beginning is critical for success. HMG VT depends on building broad-based ownership of the system across service sectors to leverage resources and improve linkages in communities.

  • CONTINUOUS QUALITY IMPROVEMENT

    Continuous Quality Improvement (CQI) refers to a methodology that attempts to ensure the success of programs and services through ongoing analyses and modifications in response to lessons learned. HMG VT has Implemented a CQI committee of staff members who represent each component of the HMG system. The committee regularly reviews policies and procedures, feedback on data collection and intake procedures, in-house training needs, and updates on statewide initiatives or changes that may affect the system.

Early detection and connection to services lead to the best outcomes for children with developmental or behavioral challenges. But too often, children don’t get connected to services they need at an early age when the benefit of those services is greatest. Many initiatives in Vermont provide services to young children, but families, child health providers, early care and education providers and others don’t always know these programs exist or how to connect with them. Help Me Grow Vermont addresses this by:

 

  • providing a centralized access point for information and referral and linkage to services
  •  
  • promoting parent engaged developmental monitoring and screening for all children
  •  
  • training providers on use of the Universal Developmental Screening Registry
  •  
  • building collaboration among community programs
  •  
  • educating and informing the community
